At Transitions Veterinary Care, we understand that you and your pet may go through different periods where you require more or less attention and support—health isn’t always a linear process. Sometimes your pet may go through a change that requires more intensive care and sometimes things resolve and your pet and you need less care.

The goal of our CARETM Plans is to ensure that you always have the optimal amount of in-home support and virtual medical care. As things change, we are here to change with you.

Our CARE™ Plans are a way for us to provide the needed care and support to both you and your pet. CARE™ Plans are selected based on your pet’s needs, your needs, and the recommendation of your Veterinarian. We believe in building trust and strong relationships to comfort you and your pet through life’s transitions.

CARE Cycle

Visits are every 3-6 months (and are not included in the monthly membership)—as determined by you, your pet’s needs, and TVC.

Included in the Continual CARE Plan

  • Access to Transitions CARETM Team
  • Visits are not included in monthly membership
  • Expected communication needs: Monthly
  • Limited Virtual Medical and Supportive Care (up to 45 minutes per month) including:
    • Nurse/doctor consultations and questions via email/phone/text
    • Medical direction for symptom care, interactions and dosing/ adjustments of medications and supplements
    • Prescription Support:
      • VetSource Prescription Portal
      • Purina Prescription Food Portal
      • Manual Prescriptions Refills

Membership

Please note that billing occurs at the end of each CARE™ Cycle during your next appointment.

  • $75 per month for the initial patient
  • $45 per month for each additional family pet

We know life can be unpredictable, which makes it hard to guess which plan might be best for you and your pet beforehand. As a result, we will assess the amount of need required during each current CARETM Cycle and will communicate to you and your family along the way which plan would be best suited. This also means you and your pet might fluctuate between different CARETM Plans each cycle to best accommodate your needs and only pay for what you use.

    • Medical direction for symptom care, interactions and dosing/adjustments of medications and supplements
    • Prescription Support:
      • VetSource Prescription Portal
      • Purina Prescription Food Portal
      • Manual Prescriptions Refills
  • 10% discount on all additional visits, supplies, and services

Membership

  • $355 per month for the initial patient
  • $305 per month for each additional family pet (plus, additional $60 discount if TVC monthly visit is combined by the same provider)

  • 15% discount on all additional visits, supplies, and services

All of Transitions CARE™ Plans are carefully designed to meet the expected needs of our families in the various stages of care that they may require. In situations where pets require an increased frequency of visits, your CARE™ plan will also increase, along with the amount of communication support included in each CARE™ Plan.

In situations where pets require an increased frequency of visits, your CARE™ plan will also increase, along with the amount of communication support included in each CARE™ Plan.

Our clients repeatedly tell us that they very much value the level of CARE™ that Transitions provides. 

Unlike conventional Veterinary Practices who deliver care through examination visits, we also provide significant medical support to you and yours in between each of our visits. We offer this medical and supportive care through multiple channels including phone calls, email, and video and text messaging. You can be confident knowing that you have our support in your back pocket when you need us most.

Here are just a few ways we are eager to support you and yours in between visits:

  • I noticed that Sammy has an upset stomach since starting his new meds yesterday, should I still give it to her?
  • I heard there was this great new product for Bella’s condition. Would it be a good fit?
  • Tucker is getting more stiff and sore in the morning, should I increase his medication? Or something different?
  • We will be travelling with Missy next week, anything I can do about car sickness?
  • Daisy is getting thinner/fatter, should I change her food? Or give different amounts?
  • What do you think is the best flea medication for Ginger?
  • What does this value mean in the lab results?
  • I can’t get Oliver to drink enough, what should I do?
  • Precious just doesn’t seem right, what do you think?
  • I am headed to the emergency room with Muffit. Will you be able to talk to the ER Doc?
  • I don’t understand what the specialist was telling me. Would you be able to contact the specialist and then discuss with me the best way to proceed?
  • The pet in the highest CARE program is charged full price
  • Any additional pets qualify for the “Additional Pet Pricing”
  • If for any reason, there is a change in the number of pets or the levels of CARE™, then the pricing will be modified to reflect the new situation.

At Transitions, the service that we provide is a model of comprehensive continual care. If that’s not a good fit for you, we completely understand and are happy to provide your pet’s evaluation and Care Summary to your primary care Veterinarian. 

While most of our clients only see the work we do during visits, the reality is that there are very significant back office operations that make sure we can support you and yours during visits and in between visits as smoothly and efficiently as possible. Often, if the work is invisible to you, it means we’ve done our job well!

Here are just a few of the jobs we are doing behind the scenes:

  • We create and maintain detailed records of your pet’s CARE Visits and Ongoing Medical Management. In addition to all of the notes that are generated by Transitions, we often collect a wide variety of external medical records including lab results, imaging reports, specialty consultation findings, prior medical histories. Because Transitions believes in multi-disciplinary support, it may also be necessary to package and share specific portions of these records with other providers. Given the complex conditions of many of the Transitions patients, this can lead to a considerable amount of data management.
  • Our back-office team writes up lab requests, properly prepares and packages lab specimens, tracks the requests, does an initial review to ensure all the results were received properly, and submits the information to Dr. Pasek and the medical team for review/analysis. Dr. Pasek and the medical team then reach out to you with their recommendations.
  • Also, some of what we call Admin support, is also very much family and patient focused. We communicate regularly about setting visit appointments and follow-ups. In addition, we want to ensure an open line of communications and so our team will often reach out to you to see how things are going in between visits. If everything is going well, this is all documented.  And if you have any questions or concerns, it’s a great opportunity to pass along your concerns to the medical team for a response. With a Transitions CARE Plan, our job is to make sure you always feel supported, every step of this journey together.

Yes, the following services are available outside of a Transitions CARE™ Plan Membership:

Stand Alone Services*:

  • Initial Care Visits
  • In-home Euthanasia
  • Consultations for second opinions and more
  • Assessments such as Quality of Life and Pain Assessments

*Note that all of these services are appointment specific and do not include any ongoing care or communication support.  In the case of consultations, your primary care veterinarian will be responsible for all ongoing care including prescription provisions and medical questions.
