At Transitions Veterinary Care™, our mission is to maximize the comfort and minimize the suffering for both pets and their caregivers.

We understand that experiencing aging, chronic illness, and cancer can be a challenging time for both you and your pet.

As Ventura County’s first certified hospice and palliative veterinary services, we provide empathetic, personalized veterinary care for pets and their people who need extra attention and care during the transitions of the pet’s life.

We take a collaborative team approach including your primary care doctor, nurses, specialists, and an experienced palliative care team to find a unique solution for every pet.

It’s our commitment to help you and your pet by providing quality veterinary care and education in the comfort of your home, allowing for the assessment of your pet in their most natural environment: their home.
